Curbside COVID-19 Evaluation/Testing
1. Schedule Appointment (Yellow Button) or Walk-ins Are Welcome
2. For NEW PATIENT - fill out Online Registration Form prior to your arrival (Red Button)
3. Park in one of the designated parking spots marked #1 through #6. Please park in any available spots if #1 to #6 are all taken.
4. Check in with front desk and tell us your car color/parking spot #. You will be given a COVID-19 Evaluation form to fill out in your car. A completed form is required for each patient.
5. A medical staff will come to you, collect necessary vitals/info/form, and perform the nasal swab. The result will be emailed to you within 1-2 hours.

COVID-19 Testing Options
Rapid Antigen Testing

Nasal swab testing to detect the presence of the virus at the time of testing

In-office with results in 15 minutes.

For patients with insurance that we are in-network with, usually there is no cost for Rapid Results Antigen Testing.

However, if you have an insurance that we are not affiliated with, the cost is $175 for the test.

For self-pay patients, the cost is $175.
Rapid PCR / Molecular Testing

Nasal swab testing to detect the presence of the virus at the time of testing

Results in less than 1 hour.

Ideal for flying / international traveling where molecular testing result is required.

For self-pay patients, the cost is $200.
PCR / Molecular Testing

Nasal swab testing to detect the presence of the virus at the time of testing

Send out to lab with results in 2-4 business days.

For patients with insurance, there is no cost for PCR lab testing.

However, if you have an insurance that we are not affiliated with, the cost of the visit is $85 and the cost of the test is billed to your insurance by the lab.

For self-pay patients, the cost is $200.
Antibody Testing

A quick blood test to determines if you have been exposed to the virus in the recent past

IgM antibodies usually appear 3-5 days after exposure at the earliest.

IgG antibodies usually appear 10-14 days after exposure at the earliest.

In-office with results in 15 minutes.

This rapid in-office test is cash-pay only; $75 for IgM/IgG antibodies testing.
